In SUPPORTIVE HOUSING, consumers are
able to access attendants on a 24-hour basis. Although
the attendant services offered at the location are subsidized, consumers
are responsible for all other expenses, including rent, utilities,
food, etc.

ELIGIBILITY
CRITERIA:
Eligible or
possess a valid health insurance
card.
Be 16 years of age
and older
Have a permanent
physical disability and require personal support services and
homemaking on a continuing basis in order to remain in the community
Be able to direct
their own personal support and homemaking services
Be able to have
medical/professional needs met by existing community health
network.
Require services
throughout a 24-hour period and be unable to have their services
effectively delivered on a visitation basis because:
- they
require monitoring or services during the night
- they
need emergency response services
- their
determined level of personal support services and essential
homemaking warrants on-site services
- those
requiring mental health services will be eligible if their mental
health services are already in place.
