The Outreach program enables people with physical disabilities to lead full, active and independent lives in Waterloo Region.
Our versatile Outreach attendants can provide service at home, at work, at school, or out in the community, and consumers of attendant services can set their own schedules and decide how they want to receive services.
Outreach services are pre-scheduled for each consumer, up to a maximum of 21 hours per week.
Our attendants can provide assistance with a wide range of activities of daily living, all of which are directed by the consumer. These services can include:
- morning and bedtime routines
- bathing/showering
- washroom assistance
- transferring
- range of motion exercises
- meal preparation including bulk meal preparation
- light housekeeping
- grocery shopping
The Outreach program is fully funded by the Ontario Ministry of Health & Long Term Care, and there is no cost to the recipient.
To be eligible for attendant services through the Outreach program, the prospective consumer must meet certain requirements. Anyone is eligible to apply for the program, as long as they answer “yes” to all of the following questions:
- Are you a person with a permanent physical disability?
- Do you require personal care?
- Are you 16 years of age or older?
- Do you have a valid Ontario Health Card?
- Are you able to direct your own attendant services?
To apply for Outreach services, contact Program Support (leanne@ilcwr.org), fill out an Outreach Application or view the Applicant Guidelines. Applications are reviewed and ranked by an Admissions Committee.
If a waiting list arises, prospective consumers will be ranked in priority order according to their needs.
